From San Juan del Sur, Ometepe is a same-department lake day: drive to the San Jorge ferry port near Rivas, cross Lake Nicaragua to Moyogalpa, then decide if one afternoon is enough or you need a night on the island. That is the trip shape. Ferry boards and road time still move. Confirm the morning you go.

This is the next piece in the 2026 planner after airport (Managua or Liberia), town (San Juan del Sur or Popoyo), beach (Remanso or Maderas), the first-week rhythm, and dry or green season. Ometepe is the inland pin most Pacific-coast travelers add once the surf week is set.

Two mountains in a freshwater sea

Ometepe sits in Lake Nicaragua (Cocibolca), in the Rivas Department — the same department as San Juan del Sur. Wikipedia’s summary: an island formed by two volcanoes, name from Nawat ome (two) and tepetl (mountain), “two mountains.” It is the largest island in the lake.

The two cones are Concepción and Maderas, joined by a low isthmus. Wikipedia’s island box lists about 276 km², roughly 31 km long, and 5 to 10 km wide. Concepción is the high point at 1,610 m (5,280 ft) and is active; the page describes a symmetrical cone and continuous eruptions that built that height. Maderas is the smaller volcano at 1,394 m (4,573 ft). Wikipedia: it has not been active in historical times, and its crater holds a crater lake. Cloud forest grows on its slopes — rare on Nicaragua’s Pacific side.

Name collision warning: Volcán Maderas on Ometepe is not Playa Maderas north of San Juan del Sur. Different place, different trip. If you already chose Remanso or Maderas for surf, that beach choice is still on the Pacific. Ometepe is the lake.

Wikipedia notes regular ferries and lanchas several times a day between San Jorge (Rivas) and the island, and that most visitors use San Jorge to Moyogalpa. I am not printing a timetable or a ticket price. Those pages go stale. Ask at the dock the morning you travel, or use a current local ferry source you trust.

Moyogalpa is the northwestern harbor town. Altagracia sits on the northeastern side. Wikipedia treats them as the island’s two municipal centers. For a day trip, Moyogalpa is the usual landing. For an overnight, pick lodging first, then which port fits that side of the island — schedules and ports still need a live check.

How the San Juan del Sur → Ometepe day actually runs

San Juan del Sur and San Jorge are both in Rivas. The corridor is: leave the Pacific bay town, reach the lake ferry at San Jorge, cross to Ometepe, reverse later. I am not inventing drive minutes. Traffic, rain, and where you start in SJDS all move the clock. Build slack the way you already built slack for an airport transfer.

On the water, plan for a real crossing, not a five-minute hop. Third-party ferry pages often quote something on the order of about an hour Moyogalpa-bound — treat that as planning language, not a promise. Wind on a big lake can change the feel of the ride. If anyone in the group gets queasy on boats, pack for that.

Cash helps at docks and small stands. I am not printing a tourist-tax dollar amount. Bring ID the way you would for any domestic Nicaragua transfer, and confirm what the ferry operator wants that day.

What Ometepe is for (and what it is not)

Ometepe is volcano silhouette, freshwater shore, and agricultural island life — plantains and livestock still matter on Wikipedia’s economy line. Tourism sits on top of that. It is not a substitute for Remanso lessons or a Maderas Pacific swell day. It is the inland contrast day.

If you overnight, the common draw is time under the cones: views of Concepción’s symmetry from the lake or lowland roads, and longer approaches toward Maderas if you want cloud-forest hiking. Wikipedia’s Maderas page says guided climbs are strongly encouraged by local guiding companies, and that the round trip can run long (on the order of many hours) with steep, muddy sections in rain. I am not selling a summit. If hiking is the point, hire a guide on the island and start early. Confirm trail conditions the morning you go — green season rain turns volcanic mud into a different sport.

Skip inventing a “top 10 Ometepe” list with hours and entrance fees. Named springs, lagoons, and kayak put-ins change access rules. Ask your hotel or a current island source. The volcanoes themselves are the durable fact.

Wikipedia’s tourism note (pre-COVID 2019 figure on that page): most visitors arrive mid-November through mid-May — which overlaps the Pacific dry season window from the season post. That is arrival seasonality, not a weather guarantee for your Tuesday.

Fit it into a San Juan del Sur week

Use the first-week planner first. Count mornings after the airport transfer and before checkout. A day-trip Ometepe burns one full day. An overnight burns a surf morning on each end unless you place it at the start or end of the stay.

If the week is a first surf week, protect Remanso mornings. Current Surf Ranch packages put lessons at Remanso, not at Pacific Maderas, and not on Ometepe. Lake day is the bonus pin, not the lesson plan.

Season still matters for the drive and the trail, not only for the beach. Dry season often means easier roads and clearer cone views. Green season means greener hills and a wetter ferry-day gamble. Same two-season coast logic as the dry vs green piece — then check that day’s forecast.

Before you spend a Pacific morning on a lake ferry, write one line: day trip or overnight. Then write the ferry check for that morning. If the line is “maybe if we have time,” keep the surf week and leave Ometepe for a longer stay.

San Juan del Sur is the Pacific base. Ometepe is the two-mountain island in the same department. Different water. Same trip if you plan it on purpose.
